About the Role
As a reablement lead assistant, you will report to the Reablement Lead and will be responsible for:
Monitoring referrals and ensuring turnaround deadlines are met.
Assessing an individual's needs and goals upon discharge from hospital.
Creating reablement plans to support the team's delivery of service.
Liaising with Health and Social Care professionals to gain the best outcome for our customers.
Working directly with the Reablement Lead, escalating any concerns or complaints if necessary.
Overseeing and monitoring equipment requests.
Being a point of contact for the reablement team, clients and outside agencies.
Completing the daily rostering for the reablement visits.
Recording any Client changes on the system.
Monitoring and imputing data.
About You
This is a fast-paced, rewarding role that could be perfect for you if you are:

Enthusiastic and able to work under your own initiative.
Excellent at problem-solving.
Passionate about ensuring deadlines are met.

A car driver with your own vehicle.
Experienced working with IT systems.
Reablement experience not essential.
