We have an exciting role on offer within our Adult Social Care and Health service as a Transition Social Worker/Co-ordinator.

This is a pivotal role supporting young people as they move from Children’s Services into Adult Social Care, ensuring their transition into adulthood is timely, coordinated and centred on what matters most to them. Working closely with the Practice Lead and Principal Social Worker, you’ll help drive high-quality, strengths-based practice across systems and partners, improving outcomes for young people and their families.
About the role:
This is a non-case-holding role with an emphasis on quality assurance, coordination, system leadership and direct work alongside practitioners, particularly where eligibility, capacity or pathways are complex or unclear.
You will:
Act as a key link between Children’s Services, Adult Social Care, Education, Housing, Commissioning, Finance and Health partners. Identify young people who need transition planning and ensure no one is missed Support and assure high-quality transition assessments and planning champion co-production, ensuring young people’s voices shape decisions and services lead on transitions audits, training and service development promote best practice in transitional safeguarding and preparing for adulthood principles.
What you’ll be doing:
Supporting Children’s Social Worker’s with assessments, planning and eligibility where adult pathways are unclear. Working directly with young people and families to build confidence, understanding and long-term aspirations. Coordinating multi-agency transition meetings, including SEND and academic transition forums and working with commissioning, housing and providers to plan future care, placements and accommodation.
Delivering training and guidance to practitioners and partners contributing to quality assurance, service improvement and policy development across transitions. Ensuring safeguarding risks are identified, shared and managed appropriately.

We expect our experienced Social Workers to hold BIA (Grade K and L), Practice Educator or AMHP (Grade L) qualifications as outlined in our progression criteria.
