The Vacancy
Our Community Teams provide compassionate, person-centred care to people living with progressive and life-limiting illnesses across Stoke-on-Trent & North Staffordshire. Working alongside Clinical Nurse Specialists (CNS) to deliver holistic, responsive, and person-centred care to patients in the community.
As a Senior Health Care Support Worker, you’ll play a key role in coordinating day-to-day patient contact within a community geographical team. This role requires excellent communication skills, confidence working independently, and a caring, organised approach to supporting patients, families and colleagues.
You’ll contribute to telephone support, home visits for non-complex patients, and the smooth running of the community service. This role is ideal for an experienced Health Care Support Worker who is passionate about palliative care and is ready for a new role, new challenges and to progress within a supportive, values-led organisation.

About You
We’re looking for someone who is compassionate, reliable and confident working in the community, with experience supporting patients and families during challenging times.
You’ll bring:
A Level 3 qualification, palliative care training or telephone review experience would be an advantage, but we’ll support development where needed.

Salary: £24,852.66 per annum
Hours: 37.5 hours a week (part time considered)
