About the role

This role is vital in the delivery of the `help when you need it` (HWYNI) support service in Bristol, which provides targeted short term interventions for adults aged 55+, living in the community, who have support needs,  to prevent and delay the need for longer-term reliance on formal services across adult social care, health and housing. 

 

The role will also focus on:

  • Supporting adults who have been admitted into hospital, working closely with hospital discharge teams to ensure that the discharge is as smooth as possible, and also use interventions to try and reduce hospital admissions.

  • Delivering one to one support to adults who are 55+ living in the community,  in a way that promotes independence.

  • Working closely with older people, who are 55+ and have support needs including mental health and substance misuse issues.

  • Providing practical support as identified in the clients support agreement, such as: ensuring maximisation of benefit entitlements, advice on budgeting and managing finances, accessing other appropriate services, dealing with correspondence, applying for grants , hoarding etc.

  • Providing support to find appropriate housing where necessary and assistance to move on and provide resettlement activity.

  • Liaising with family, other professionals, agencies and organisations, as necessary, (this may include negotiating with debt agencies on the clients behalf, on occasions it may be necessary to attend court and/or deal with bailiffs to support the client), in order to maintain clients ability to remain living independently.

  • Regularly review clients' support plans and participate in review meetings.

 

About you

An enthusiastic and motivated individual, you will be passionate about providing a high level of service. You’ll need to have a good knowledge of welfare benefits so you are able to provide advice to clients. 

Alongside this you will need a full driving licence and your own transport as you will be required to travel to support our customers within their own homes. 

Ideally, you will have experience in providing support to vulnerable adults but this is not essential.
